"Leaky gut" is not a recognized medical diagnosis in traditional allopathic medicine, but "intestinal permeability" is a well-documented physiological phenomenon.
The Mechanics of the Intestinal Barrier
Your intestinal lining covers approximately 4,000 square feet (about the size of a tennis court), yet it is only a single cell layer thick. This epithelial layer has a dual, conflicting mandate: absorb nutrients efficiently while keeping antigens, toxins, and pathogens out of the bloodstream.
The cells (enterocytes) are bound together by proteins called tight junctions. In a healthy gut, these tight junctions are heavily regulated gatekeepers. In increased intestinal permeability, these junctions widen, allowing lipopolysaccharides (LPS) and undigested food particles into the systemic circulation, triggering an immune response.
Zonulin: The Modulator
Zonulin is a protein synthesized in intestinal and liver cells that reversibly regulates intestinal permeability. Discovered by Dr. Alessio Fasano in 2000, it is the only known physiological modulator of intercellular tight junctions.
Elevated zonulin levels—often triggered by gliadin (a component of gluten) or intestinal bacterial dysbiosis—widen the tight junctions. This is why testing companies like Viome look for markers of inflammation and dysbiosis that precede elevated zonulin.
Diagnostic Testing
You cannot diagnose leaky gut by symptoms alone, as bloating, fatigue, and brain fog are highly non-specific. Clinical validation requires measurement.
- Lactulose/Mannitol Test: The gold standard. You ingest two sugars (one large, one small). The ratio of these sugars excreted in urine indicates the degree of permeability.
- Zonulin Testing (Serum or Stool): While helpful, stool zonulin is highly volatile and less reliable than serum measurements.
- Comprehensive Stool Analysis: Tests like Thorne provide indirect evidence by showing the absence of SCFA-producing bacteria, which are necessary to maintain the mucosal layer.
